"Have (Mid-East talks) failure killed two-state goal?" The word "failure" is singular. With the qualification "Mid-East talks", it forms the subject of this sentence. A singular subject calls for a singular verb form ("Has"). Probably the writer got confused by the plural "talks". But hey, this is the BBC. They can do better than this.
